Maybe. Another possibility is that a module with dependencies/links to another module gets edited and the dependencies aren’t also edited to conform. I don’t know WordPress, but it’s a common problem in other software systems.

Yes, you’ve got it. “Em” is for “emphasized,” which FYWP outputs as italics. The <i> tag (for italics) is not currently recognized by FYWP (although it was previously).
HTML formatting tags start with <something> and end with </something>. The slash denotes the closing of the code.
ETA: “FYWP” stands for “Fuck you WordPress,” which is the particularly vexing mutation of the blogging platform WordPress that is used here at Balloon Juice.
