The quote is from the former, and the image is of the latter. The Libertarians to whom the tweet is directed apparently have little understanding of the matter also.

@WaterGirl: Explanation above does well, but what makes the joke extra funny is that the philosopher Hayek, author of The Road to Serfdom, is someone who right-wingers claim as their own. Libertarians are known to respond “Obviously, you haven’t read Hayek or you would understand why (absolutely true observed thing) is impossible!” Hayek gets waved around as an “I win this argument!” card a lot. So jokes dinging him are extra sweet, and likely to bring out the butthurt dudes.
Also, the actual Hayek gave healthcare as something that should rightfully be a government function, so all the fanboys should just shut up already.

@WaterGirl: Friedrich Hayek is an economist-philosopher who championed an extreme free-market economic style. He is a favorite of libertarians for his totally hands-off philosophy. He is often quoted by right-wingers trying to dis “socialism” in this country. If the right is against something the government does, chances are they have a quote from Hayek to back them up. Salma Hayek is not really his opposite, but close enough to be a joke. The other joke is that the people who read the tweet actually thought she said it, and reacted the way you would expect right wing misogynists to react.
