… The WTO’s 164 members on Friday discussed the “outcome document” that stems from months of negotiations between the main parties – the United States, the European Union, India and South Africa – in an effort to break an 18-month deadlock over the issue.

“It went very well and here’s why I say that. No member rejected the outcome as completely unacceptable,” Ambassador Lansana Gberie from Sierra Leone, who chairs the council tasked with finding an agreement on the waiver, told Reuters after the closed-door meeting…

However, two Geneva trade sources said after the meeting that some delegations had said the proposal fell short since it was too narrowly focused on vaccines, echoing criticism from some civil society groups.

And while China voiced broad support for the document it also raised an objection to some of the wording that appeared to exclude it from the waiver on the basis of its global share of vaccine exports, the two sources said…

The waiver idea, proposed by India and South Africa in October 2020, is supported by the majority of members of the global trade body.

But some wealthy countries including Britain and Switzerland have in the past raised objections on grounds that it could harm pharmaceutical research…

WTO Director-General Ngozi Okonjo-Iweala, who has been involved in brokering the talks and wants a deal by the June ministerial conference, says an agreement would be “hugely important”.

The new draft deal, which has unresolved areas, must pass by consensus and any member of the organisation has the right to a veto…

We could run out of vaccine doses just as a new, potential end-of-summer variant emerges. It’s the weekend, take the time to read the whole thing:

The White House could run out of Covid-19 vaccines if it moves forward with plans to encourage all adults to get a second Covid-19 vaccine booster dose by roughly Sept. 1, according to a tranche of budget documents sent to Congress that have not previously been made public.

Although Food and Drug Administration officials have hinted that all American adults may be encouraged to get second boosters this fall, right now, second booster doses are only available to people over the age of 50. The budget documents make it clear that if the administration does want to push second boosters, it will need more money to make it happen: it needs at least 87 million more vaccines for adult boosters, and another 5 million more for first boosters for kids…

The documents, which STAT is making public in full for the first time, offer a window into the Biden administration’s planning for the next phase of the Covid-19 response, including contingency planning for the purchase of vaccines and therapeutics. It also reflects how the Omicron surge ravaged the Covid-19 response budget.

The insights into the current state of Covid-19 funding come as the White House’s ongoing efforts to get more money are stalling. A bipartisan deal to give the administration $10 billion for the Covid-19 response has been held up over disputes about public health policy related to migrants on the southern border, and it’s unclear if the funding has a path forward…

    I’m double vaccinated and double boosted and I came down with a breakthrough case a few days ago.  Enough to keep me bedridden for two days with violent chills, a pounding headache, cough, upper respiratory congestion.

    If I hadn’t been vaccinated I wouldn’t be sitting here right now feeling much better.

    ALBANY — COVID continues trending upward in the Capital Region, with the reported rates of infection and hospitalization both more than triple the rates at the start of April.

    New infections reported each day and hospital admissions remain far below the numbers seen in the early January surge four months ago. But they are still rising, and they are contributing to upstate New York’s status as a hotspot in a nation where the pandemic appears to be subsiding.

    New England, New York and New Jersey lead the nation in daily new infections, the federal Centers for Disease Control and Prevention reports, and they contain almost all 79 counties nationwide rated by the CDC as having a “high” community level of COVID.

    In the Capital Region, that translates to 189 people hospitalized with the disease as of May 5 and an average of 52.4 known new infections per day per 100,000 people.

    Malaysia’s Ministry of Health reported 1,251 new Covid-19 cases yesterday, for a cumulative reported total of 4,455,364 cases. It also reported seven deaths for an adjusted cumulative total of 35,576 deaths – 0.80% of the cumulative reported total, 0.80% of resolved cases.

    There were 25,517 active cases yesterday, 1,937 fewer than the day before. 1,026 are in hospital. 76 confirmed cases are in ICU; of these patients, 39 confirmed cases cases are on ventilators. Meanwhile, 3,181 more patients have recovered, for a cumulative total of 4,394,271 patients recovered – 98.6% of the cumulative reported total.

    1,242 new cases reported yesterday were local infections. Nine new cases were imported.

    The National Covid-19 Immunisation Programme (PICK) administered 19,105 doses of vaccine on 6th May: 4,382 first doses, 13,214 second doses, and 1,509 booster doses. The cumulative total is 70,365,518 doses administered: 27,779,984 first doses, 26,758,696 second doses, and 16,040,212 booster doses. 85.1% of the population have received their first dose, 81.9% their second dose, and 49.1% their booster dose.

    Sorry I haven’t posted an update from the UK for a few days. Real life intervening! On an unrelated note, the Tories got a right kicking in Thursday’s local elections, losing 300 seats on local councils. There are a lot of very unhappy ex-Conservative councillors out there!

    Anyway, here are the figures from Friday, where we had 11,076 new cases. The rolling 7-day average is down by 28%. New cases by nation,

    England – 9405

    Northern Ireland – 316

    Scotland – 1226

    Wales – 129.

    Deaths – There were 228 deaths within 28 days of a positive test yesterday. The rolling 7-day average is down 16.7%. 203 deaths were in England, 1 in Northern Ireland, 18 in Scotland and 6 in Wales.

    Testing – 281,388 tests took place on 5th May. The rolling 7-day average is down by 16.2%.

    Hospitalisations – There were 10,763 people in hospital and 253 on ventilators on Thursday, 5 May. As of 2nd May, the 7-day average for hospital admissions was down by 25.4%.

    Vaccinations – Now only updated weekly. The next update is due Monday.

    This makes me nervous about the folks coming to remodel my master bathroom next week. It’s still not warm enough around here to have all the doors and windows open for ventilation. I’m “vaxed to the max” but I’m worried it won’t be enough.

    Did you consider asking your doctor for paxlovid? That’s my plan should I start coming down with symptoms and test positive (we have a little stash of tests).

    We’re going to our first super-spreader event this afternoon, Ohio Son’s community college graduation. Not looking forward to it, I’ve been to graduations at this areana before, the acoustics are terrible and all the speakers are unintelligible. Not that graduations aren’t terribly boring when you can hear everything that’s being said.

    Seems to me an awful trade-off, risking Covid for several hours being uncomfortable with the risk of being a pandemic statistic but what can I do?

    Guangdong Province reported 8 new domestic confirmed (1 previously asymptomatic, 6 at Guangzhou & 2 at Zhanjiang) & 1 new domestic asymptomatic (at Guangzhou) cases. 1 of the cases at Guangzhou was previously asymptomatic, the rest are traced close contacts under centralized quarantine. 1 of the cases at Zhanjiang is a cold chain logistics worker found via regular screening, & the other is a traced close contact. 9 domestic confirmed & 3 domestic asymptomatic cases recovered. There currently are 157 active domestic confirmed & 40 active domestic asymptomatic cases in the province. As the province does not break down recoveries by administrative divisions, I cannot track the count of active cases in administrative divisions, given the multiple simultaneous outbreaks. 1 business at Zhanjiang is currently at Medium Risk.

    Beijing Municipality reported 45 new domestic confirmed (4 previously asymptomatic, all mild) & 8 new domestic asymptomatic cases. Of the 66 domestic positive cases reported between 3 PM on 5/5 & 3 PM on 5/6, 2 were from community screening, while the others are persons under centralized quarantine or residents under movement control. As the city does not break down recoveries by imported vs. domestic cases, I cannot track the count of active domestic cases there. 20 sites are currently at High Risk. 34 sites are currently at Medium Risk.

    Shanghai Municipality reported 253 new domestic confirmed (175 previously asymptomatic) & 3,961 new domestic asymptomatic cases. 4,021 of the new domestic positive cases were already under quarantine & 18 via screening of persons deemed at risk of exposure. There were 13 deaths (avg. 83.8 y.o., youngest at 62 y.o. & oldest at 95 y.o., all w/ a range of underlying conditions, none vaccinated). 1,663 domestic confirmed & 11,626 domestic asymptomatic cases recovered. There currently are 7,699 active domestic confirmed (464 serious & 87 critical) & 71,268 active domestic asymptomatic cases in the city. 13 sites are currently at Medium Risk.

    Guizhou Province reported 1 new domestic asymptomatic (at Tongren) case, a person coming from Shanghai & under centralized quarantine since arrival. There currently are 1 active domestic confirmed (at Qiandongnan Prefecture) & 3 active domestic asymptomatic (all at Tongren) cases remaining.

    Jiangxi Province reported 10 new domestic asymptomatic cases. 34 domestic confirmed & 34 asymptomatic cases recovered. There currently are 111 active domestic confirmed & 570 active domestic asymptomatic cases in the city. As the province does not break down recoveries by administrative divisions, I cannot track the count of active cases in administrative divisions, given the multiple simultaneous outbreaks.

    • Nanchang did not report any new domestic positive cases. 7 domestic confirmed & 8 domestic asymptomatic case recovered. There currently are 18 active domestic confirmed & 5 active domestic asymptomatic cases in the city..
    • Fuzhou reported 1 new domestic asymptomatic case, a person coming from elsewhere & under centralized quarantine.
    • Shangrao reported 9 new domestic asymptomatic cases, 12 are traced close contacts under centralized quarantine & 4 via screening of persons in high risk occupations. 27 domestic confirmed & 26 domestic asymptomatic cases recovered. There currently are 94 active domestic confirmed & 517 active domestic asymptomatic cases in the city.

    Henan Province reported 29 new domestic confirmed & 69 new domestic asymptomatic cases. 4 domestic confirmed & 3 domestic asymptomatic cases recovered. There currently are 101 active domestic confirmed & 264 active domestic asymptomatic cases in the province. As the province does not break down recoveries by administrative divisions, I cannot track the count of active cases in administrative divisions, given the multiple simultaneous outbreaks.

    • Zhengzhou reported 14 new domestic confirmed & 25 new domestic asymptomatic cases, most are persons under quarantine.
    • Xuchang reported 12 new domestic confirmed & 30 new domestic asymptomatic cases, many of the cases are backlogs from 5/3 – 5/5.
    • Zhoukou reported 8 new domestic asymptomatic cases, 2 traced close contacts under centralized quarantine & 6 via community screening.
    • Xinyang did not reported any new domestic positive cases. 2 sites are currently at Medium Risk.
    • The rest of the province reported 3 new domestic confirmed (1 each at Kaifeng, Pingdingshan & Shangqiu) & 6 new domestic asymptomatic (2 each at Puyang & Pingdingshan, & 1 each at Luohe & Shangqiu) cases. 2 of the cases at Pingdingshan are persons recently returning from Zhengzhou & Xuchang, respectively, & the 3rd is a traced close contact, all under centralized quarantine since 5/5. Both cases at Puyang were found via community screening. The cases at Shangqiu & Kaifeng are traced close contacts under centralized quarantine. The case at Luohe is a traced close contact of domestic positive cases reported by Xuchang & under centralized quarantine.

    Sichuan Province reported 3 new domestic asymptomatic (2 at Bazhong & 1 at Dazhou) cases, all persons coming from elsewhere & under centralized quarantine. As the province does not break down recoveries by imported vs. domestic cases, I cannot track the count of active domestic cases there.

    At Yinchuan in Ningxia “Autonomous” Region the domestic asymptomatic case recovered.

    Chongqing Municipality reported 1 new domestic confirmed case, a person under centralized quarantine. There currently are 7 active domestic confirmed & 1 active domestic asymptomatic cases remaining in the city.

    Qinghai Province reported 1 new domestic asymptomatic case, at Xining, found via regular screening of persons in high risk occupations. 1 domestic confirmed case recovered. There currently are 12 active domestic confirmed & 6 active domestic asymptomatic cases in the province.

    Yunnan Province did not report any new domestic positive cases. 1 domestic confirmed & 13 domestic asymptomatic cases recovered. There currently are 4 active domestic confirmed & 67 active domestic asymptomatic cases remaining in the province.     • Beijing Municipality – 1 confirmed case, coming from Hong Kong; 1 asymptomatic case, no information released

    Overall in Mainland China, 1,878 confirmed cases recovered (8 imported), 12,110 asymptomatic cases were released from isolation (65 imported) & 185 were reclassified as confirmed cases (all domestic), & 41,472 individuals were released from quarantine. Currently, there are 9,975 active confirmed cases in the country (163 imported), 587 in serious/critical condition (all domestic), 103,935 active asymptomatic cases (489 imported), 1 suspect case (imported). 444,565 traced contacts are currently under centralized quarantine.

    As of 5/6, 3,350.934M vaccine doses have been injected in Mainland China, an increase of 1.232M doses in the past 24 hrs.

    On 5/7 Hong Kong reported 278 new positive cases, 18 imported & 260 domestic (97 via RT-PCR & 163 from rapid antigen tests), 0 deaths.

    On 5/7, Taiwan reported 46,536 new positive cases, 159 imported & 46,377 domestic. There were 11 new deaths (ages ranging from 40+ to 90+, actual dates of death between 4/30 & 5/4, all having a range of underlying conditions, 7 partially/fully vaccinated).

    I thought about trying to get paxlovid but I got the impression they were reserving it for extremely high risk people. Diabetes, Immunocompromised, etc.

    I’m still coughing from post-nasal drip and my sense of smell is missing, but I’m awake, alert and feeling much better.

    My wife had the exact same symptoms a few days before me, including a high fever. We never go out unmasked, so we think we caught it from family who lives upstairs. Simply sharing the same vestibule was enough. It literally spreads like measles.

    Now I’m wondering if I’ve got “super immunity” because of the double vax and double boost and omicron I caught. That’s what some of the more excitable media outlets are saying, but I still intend to be careful

    I didn’t take anything.  I just rested and kept hydrated.

    It felt like a bad case of flu.  Lots of sleeping on and off.

    I’m glad my wife and I weren’t sick at the same time.  So we were able to help each other.

    No 91-Divoc update today, so no detailed State by State breakdown. Cases nationwide cases rose to 71,100. Hospital admissions rose to 17,039. Deaths rose slightly to 606.

    Looking at the two bellwether States, cases have risen 33% in the last week in NJ, an acceleration from 20% in the previous 7 days. New York State reports that cases also continue to rise there, up almost 30% for the week. The silver lining is that cases have declined over that period in 5 of the State’s 7 Upstate regions, and are close to doing so in a 6th. Almost all of the State’s increase is due to the 3 NYC regions.

    Flu Report for 4/24/22-4/30/22

    Flu continues high or increasing. Positivity up from 7.8% to 8.1%. Lab cases up from 3,451 to 4,421 with a whopping 4,908 added to previous weeks (by far the most this year). H3N2 continues its unprecedented dominance, with 99.7% of cases being type A and 99.2% of subtyped A flus being H3N2. Hospital admissions down from 3,243 to 3,070.

    Mortality due to pneumonia, influenza and COVID up to 7.2%, above the epidemic range. This reflects deaths due to COVID as there are few flu deaths at present.

    Flu going back up again, against all precedence, in this very odd season.

    I am starting to wonder if immunosuppression from COVID is playing a role. There haven’t been significant restriction on the population level for over 2 months, and normally flu peaks come and go faster than that. This business of plodding along at moderately high levels week after week is unprecedented.

    @germy: Weird recommendation here: I am recommending people do NOT take Paxlovid unless they are at high risk.

    Why?
    This study found 32 of 46 patients with IBD still positive for gut COVID activity roughly SIX MONTHS after infection. IBD patients may not be typical on this – but on the other hand they might be, and in any case it proves COVID-19 can set up long-standing infections. If that’s the case, many will need antiviral therapy to get rid of it, and the current 5-day Paxlovid course is manifestly inadequate for that. (Hep C needs 8 weeks). An inadequate course allows for the development of resistance.

    So I think unless you really need it, don’t use Paxlovid or any other antiviral until we have studies on the treatment protocols needed to completely clear persistent infection.

    If you are high risk, however, by all means DO use Paxlovid. Keeping your infection antiviral-sensitive does not help if you are dead!

    Wear a mask. Bring some hand sanitizer. Have a test kit handy and check yourself sometime after the event.

    @Fair Economist: I just read that being over 65 automatically makes you high risk. I am 67 and my husband is 69 and neither of us have any other qualifying conditions. I don’t want to take an rx away from someone really high risk. Would you take Paxlovid in my instance? Haven’t had Covid yet but just thinking for the future.

    @Eunicecycle: Honestly that seems kind of borderline to me. I don’t think you are keeping it from people who need it; the issues are more distribution than supply. If I did not have time to do research for exact risk status I would follow the official guidelines.

    @germy: Similar breakthrough case for me, back over New Year’s weekend. I wore my mask everywhere outside of my apartment, but my neighbors were not as considerate and I often saw them in the hallways with no masks. And sometimes I would forget to pull mine up the minute I stepped outside the door. Omicron was just insanely contagious, and the virus isn’t getting less so. The only reassuring part was that I didn’t get very sick. I’m assuming that’s because in addition to being boosted, I wasn’t exposed to a huge viral load.

    Covid finally broke into our house here in Vancouver WA.  My 16 year old daughter seems to have brought it home from school last week.  She had a mild 24 hour fever and stuffy nose and got better in 2 days.  My wife came down with typical mild flu symptoms and is feeling better.  I had a couple days of congestion and runny nose but no fever and am now feeling fine.  Basically it was all identical to a typical seasonal round of cold/flu  that we used to get every year and never remarked about.  In fact I have had many much worse bouts with the flu over the years.

    All of us were completely vaxed and boosted.  I had the most mild symptoms and I had gotten my second booster just days before (first three shots were Pfizer and most recent booster was Moderna).

    This was the first any of us had gotten Covid in 2 years of the pandemic.  Honestly it is kind of a relief that it was so mild and uneventful and I guess we are all kind of off the hook for 6 months or so.

    I expect this is kind of where we are going to end up.  Most people will get mild Covid on occasion if they keep up their vaccinations and it will be no big deal.  Covid treatments for severe cases will continue to get better and better over time.  Some people will still die.  And that will be the new reality.  It is never going completely away.

    We change mask rules and the case numbers go up. What to do? The virus becomes more contagious. It has sex with its cousin Delta and becomes more deadly and more contagious.
    I know pornography when I see it.

    My son who works on Capitol Hill just tested positive.  So far just mild cold symptoms.  He says he works in a swirling cesspool of germs.
