… More than 2½ years into the pandemic, and with a highly contagious version of the virus circulating, the CDC guidelines for what to do when falling ill — and when to return to public life — continue to stoke as much confusion as clarity. That’s a reflection of the changing nature of the virus, the inherent unpredictability of an infection, and the demands and expectations of work and home life.
With new research showing that people are often infectious for more than five days, the CDC guidance has drawn criticism from some infectious-disease experts. The Biden protocol strikes many of them as the right way to go — because it’s empirical evidence that a person isn’t shedding virus…
The CDC’s guidance has been under internal review in recent months. A revamped set of recommendations is expected to be rolled out in coming weeks, according to three administration officials and advisers who spoke on the condition of anonymity to describe sensitive internal discussions. A draft of the updated guidance at the moment does not include a requirement to test negative before exiting isolation, they said.
The existing CDC guidance says patients can end isolation five days after their first day of symptoms, so long as their symptoms have improved and they have been fever-free for at least 24 hours without fever-reducing medication. The CDC encourages people who become very sick or have weakened immune systems to isolate for 10 days…

Malaysia’s Ministry of Health reported 4,204 new Covid-19 cases yesterday, for a cumulative reported total of 4,687,470 cases. It also reported eight deaths for an adjusted cumulative total of 35,985 deaths – 0.77% of the cumulative reported total, 0.77% of resolved cases.
35,674 Covid-19 tests were conducted on 31st July, with a positivity rate of 8.4%.
There were 44,278 active cases yesterday, 386 fewer than the day before. 1,537 were in hospital. 56 confirmed cases were in ICU; of these patients, 36 confirmed cases were on ventilators. Confirmed cases nationwide declined to 121,700, still within their recent 120-130,000 range. Hospitalizations have plateaued in the past 10 days reported in the 45-47,000 range, and as of July 30 were 46,100. A commenter at Seeking Alpha who works in a hospital wrote to me that the big increase in the past several months has been people showing up with unrelated issues testing positive for COVID, I.e., “patients with COVID.” Deaths are also steady at 431, within their recent 400-450 range as well.
Regionally there has been a small decline in the West, while the other three regions are steady. NY and NJ are showing slight decreases. There are no other remarkable trends in other States.
The CDC updated its variant tracker yesterday, showing BA.4&5 making up 97% of all cases. They also included a new subvariant, BA.4.6, in their analysis, indicating it constituted 4% of all cases, or 1/3rd of the BA.4 total. It is primarily a factor in the northern Great Plains, where it makes up 9% of all cases. But it has not been particularly growing in the past month, nor does it seem to be replacing BA.5. Similarly, while a few cases of BA.2.75 are showing up in most States, they are not showing up in the CDC data at all. I have not seen any medical commentary on either subvariant in the past week.
Unless a new variant shows up imminently, I suspect we are entering a period of decline.

I don’t want to give fuel to the “vaccines are useless” fire, but if an almost octogenarian survivor of a cardiovascular event is “feeling fine” and working, as he seems to be, Joe is doing way better than I, a 4-times vaccinated mid-50’s woman in very good overall health and outstanding cardiovascular health, did. COVID knocked me out for two days, and the paxlovid rebound rendered me nearly unable to walk because of joint pain and swelling.

The WaPo article clarified a question for me. I tested positive with a faint, slowly appearing line last Wednesday after noticing symptoms during the night before. 5 days later I tested again and the Covid line showed up immediately and was dark – so I was apparently more infectious after 5 days than on the first day I tested. My symptoms have remained mild, fortunately, though I still have a cough. I got my antibody infusion that afternoon and was told that, though the home test is usually a good metric for infectiousness, for the next 90 days all results are suspect and I should rely on symptoms to see if it has recurred. The nurse and I talked about their difficulty keeping up with the guidelines and explaining those to patients who want cut-and-dried answers. It’s a new disease, people, with lots of variants!
I’ve also developed a new appreciation for the difficulty of navigating isolation. Are there multiple people in the household? Does everyone have their own bedroom and bath? Who cooks, does laundry, shops, takes care of pets, does dishes, etc. Do you test everyone everyday? Can we touch the pets? Where do all the tests come from? And when do you come out of isolation – when everyone is negative, 5 days after any symptoms? I live in a household with 5 people, a cat and a dog – where 2 people are currently positive. It’s complex. Everyone here (but the animals) are double vaxxed and the adults are all double boostered, so seeing mainly mild symptoms, but I’m not at all surprised that it just usually spreads to everyone in a household.

At Jiangsu Province 2 domestic asymptomatic cases recovered. There currently are 1 active domestic & 9 active domestic asymptomatic cases in the province. As the province does not separate recoveries by jurisdictions, I cannot track the count of active domestic cases at the different jurisdictions, due to the simultaneous outbreaks in different locations.
Zhejiang Province reported 9 new domestic confirmed (4 previously asymptomatic, all at Jinhua) & 45 asymptomatic (43 at Jinhua & 2 at Shaoxing) cases. 7 of the cases at Jinhua are traced close contacts under centralized quarantine, 40 from community screening & 1 at fever clinic. The cases at Shaoxing came from Jinhua on 8/1 & under centralized quarantine. 2 domestic confirmed cases recovered. As the province does not break down recoveries by imported & domestic cases, I cannot track the count of active domestic cases there. 3 sites at Yiwu in Jinhua are currently at High Risk. 2 sites at Taizhou & 1 at Yiwu in Jinhua are currently at Medium Risk.
Fujian Province reported 2 new domestic confirmed cases, both at Xiamen, both traced close contacts under centralized quarantine. 1 domestic asymptomatic case recovered. There currently are 11 active domestic confirmed & 2 active domestic asymptomatic cases in the province. Nanchang in Jiangxi Province did not report any new domestic positive cases. 3 asymptomatic cases recovered. There currently are 1 active domestic confirmed & 14 active domestic asymptomatic cases in the city. 2 sites are at Medium Risk.
Sichuan Province reported 1 new domestic confirmed case (previously asymptomatic), at Chengdu. 7 domestic confirmed & 4 domestic asymptomatic cases recovered. As the province does not break down recoveries by imported & domestic cases, I cannot track the count of active domestic cases there. 27 sites at Chengdu & 1 at Deyang are currently at High Risk. 43 sites at Chengdu & 2 at Deyang are currently at Medium Risk.
Chongqing Municipality reported 1 new domestic confirmed case (at Hechuan District), a person under centralized quarantine. 1 domestic asymptomatic case recovered. There currently are 14 active domestic confirmed & 3 active domestic asymptomatic cases in the city. 3 sites are currently at High Risk, & 2 are currently at Medium Risk.
Yunnan Province reported 1 new domestic asymptomatic (at Menglian County in Pu’er) cases. There currently are 1 active domestic confirmed & 49 active domestic asymptomatic cases there.
Imported Cases
On 8/3, Mainland China reported 58 new imported confirmed cases (5 previously asymptomatic), 94 imported asymptomatic cases, 0 imported suspect cases:
Overall in Mainland China, 130 confirmed cases recovered (40 imported), 640 asymptomatic cases were released from isolation (58 imported) & 11 were reclassified as confirmed cases (5 imported), & 27,872 individuals were released from quarantine. Currently, there are 1,548 active confirmed cases in the country (588 imported), 2 in serious/critical condition (both domestic), 5,903 active asymptomatic cases (635 imported), 0 suspect cases. 106,601 traced contacts are currently under centralized quarantine.
As of 8/3, 3,424.838M vaccine doses have been injected in Mainland China, an increase of 373K doses in the past 24 hrs.
On 8/3 Hong Kong added 5,020 new positive cases, 247 imported & 4,773 domestic (1,708 via RT-PCR & 3,065 from rapid antigen tests), 3 new deaths (76 – 94 y.o., only 1 boosted).
On 8/3, Taiwan added 22,959 new positive cases, 252 imported & 22,707 domestic (including 134 moderate or serious). There were 56 new deaths (ages ranging from 40+ y.o. to 90+ y.o., actual dates of death range from 5/20 – 8/1, 51 w/ underlying conditions, 14 fully vaccinated & boosted).
