ICU capacity and infection lags

Forbes reports that several regions are at minimal ICU capacity:

 

Jackson, Mississippi—the state’s capital—has “zero” ICU beds available, while the rest of the state has “very few,” State Health Officer Dr. Thomas Dobbs said.

Tulsa health officials announced Tuesday night that the city had run out of ICU beds…

Ed Yong makes a terrifying point:

There is a roughly 12-day lag between rising cases rising hospitalizations.

We need to think about lags.

  • Deaths lag ICU admissions.
  • ICU admissions lag hospitalizations.
  • Hospitalizations lag diagnostic test results.
  • Diagnostic test results lag infections.

If no one is infected today, we won’t see much change in trends on hospitalizations, ICU admissions and deaths until the first week of December.

Election day infections are starting to hit the hospitals now.   On November 3rd, Election Day, there were 86,000+ diagnosed cases.  On November 15th, a Sunday which is usually a “light” day, there were 139,000+ diagnosed cases.  We are building a huge bolus of need that has yet to actualized.  That need will be arriving on a system that is already at capacity.

We’re going to be entering a dark period for the next several weeks.  We need to drive down future ICU demand.  That means masking up.  That means not mixing with people outside of our households.  We’re entering the suck.  We can only reduce the duration of the suck.

Why are there fewer counties with zero premium bronze plans?  Is it sabotage?  Is it nefarious?

HELL NO.

Competition increases net premiums for subsidized individuals who purchase plans that are priced below benchmark.

The ACA uses a price linked subsidy system where the subsidy is a gap filler between an individual’s expected contribution which is a function of income as measured by the household federal poverty level and the benchmark plan.  The benchmark plan is the second least expensive silver plan.  When there is no competition, a monopolist, or at least a quasi-competent monopolist, will jack up their benchmark as high as possible and then flood the zone with a single cheap silver and their normally priced bronze plans.  This produces a lot of zero premium exposure.  When there is competition and multiple insurers are trying to compete on price, there is a race to minimize premium spreads between the cheapest silver and the benchmark which drives the benchmark down.  A lower benchmark level means premium spreads are compressed.  This spread compression increases net premiums for subsidized buyers.

Before silverloading, zero premium plans were rare for single and young individuals earning over 150% FPL.  With silverloading, silver premiums have spiked relative to Bronze, so zero premium plans will be more common at any point from 2018 to present and the future compared to 2014-2017.  However, as insurers return to markets that they fled in 2018 due to policy and actuarial uncertainty and insurers expand into new, monopolistic markets, zero premium plans will become rarer.

In this week’s Medium Cool, let’s talk about the environment.

As a city dweller, I find myself often craving the outdoors. And as a Midwesterner, I love the desert, the mountains, and the ocean. Yes, human beings are wrecking the planet. And yes, if we don’t do something about it, the planet is going to get even warmer and less hospitable for life.

What cultural works remind you, or teach you, how valuable and precarious nature is? I don’t necessarily mean documentaries, or Al Gore TED talks, as much as a film or a novel, maybe a painting or photography, that reattaches you to the landscape as a living, precious, vulnerable place.
