The video is 13 minutes – which seems like forever these days – but I couldn’t stop watching.

I hope there are a lot of elections deniers people quaking in their boots after seeing this.

County Clerk and election denier Tina Peters allowed MyPillow CEO Mike Lindell to use her security card to breach Dominion voting machines in the 2020 election.

The judge just sentenced her to 9 years in prison after dragging her for FILTH.

You’re really missing something if you don’t watch the whole thing.  He takes her actions apart, piece by piece and he lays it all out.  We all live for the day when a judge says these things to Donald Trump.

But in the meantime, this was riveting.

Spillovers from Medicare Cancer Care Models

In the most recent issue of the Journal of Clinical Oncology, Mullangi et al examine whether the Medicare Oncology Care Model led to spillover and practice pattern changes within participating clinics for non-Medicare patients.  This is a fascinating question.  A spillover occurs when a change in practice occurs that impacts people who are not targeted by the intervention.  We know spillovers are quite common in Medicare Advantage.  Once a county reaches sufficient Medicare Advantage enrollment (usually just under 20%) traditional Medicare costs decrease as well.  The observed logic is that doctors typically practice as if they are indifferent to insurance type so once they get told by a big enough payer to do something in a certain way in order to get paid, they’ll do that for basically everyone.

The Oncology Care Model is a bundled payment project.  It gives oncologists a set of cash to care for a cancer patient for six months. Initial research found clinical cost savings but not programmatic cost savings as these models require a lot of bonuses and administrative complexity to run.   But the initial research only looked at costs within traditional Medicare Fee for Service.  This paper looks at a broader range of payers.

So what did it do and how did the researchers try to figure this out:

 

This observational study used administrative claims from a large national payer, yielding 157,189 total patients with commercial insurance or MA with solid malignancies who initiated 229,376 systemic anticancer therapy episodes before (2012-2015) and during (2016-2021) the OCM at 125 OCM-participating practices (a subset of total OCM practices) and a 1:10 propensity-matched set of 860 non-OCM practices. We used difference-in-differences analyses to assess the association between the OCM and total episode spending…

The OCM was associated with adjusted spending decreases for both high-risk (–$6,756 USD [95% CI, –$10,731 USD to –$2,781 USD], P = .001) and low-risk (–$4,171 USD [95% CI, –$7,799 USD to –$543 USD], P = .025) episodes. OCM-associated spending reductions were strongest for outpatient (–$5,243 USD [95% CI, –$8,589 USD to –$1,897 USD], P = .002) and infused/injected anticancer drug (–$3,031 USD [95% CI, –$5,193 USD to –$869 USD], P = .006) spending. There were no associations between OCM participation and changes in hospital or ED utilization nor quality of care.

WHAT?

The short version is that costs went up everywhere BUT for the practices in the Oncology Care Model, costs for commercially insured patients went up far less and far slower than patients who were treated by docs who were not in the OCM.  There were no differences in quality of care that would make us wary of these cost savings.

Methodologically, this is a reasonable way of asking this question.

Spillovers are fascinating (the R&R I’m sending back this afternoon is a spillover paper) as most policies are not evaluated on their spillovers even if we have reason to believe that spillovers can drive a substantial portion of total results.  Furthermore, once we have evidence of spillovers happening, it changes the policy creation and modification incentives — do we change something in the primary program just to modify the spillover effect?  That is a tough question.

I think I’ll be thinking hard about this paper on my next couple of long walks.

… Not long ago, of course, J.D. Vance was widely considered a drag on the ticket (cat-ladygate, that interminable convention stemwinder, the whole “America’s Hitler” thing) with a bionic billionaire sugar daddy. (It’s hard to imagine Vance’s political rise without Peter Thiel’s checkbook and Tucker Carlson’s lobbying.) But whether or not his nimble debate performance has any impact in the polls, it certainly restored his standing in the premature but inevitable 2028 conversation. “It was a moment for him to come out of Trump’s shadow,” said one Vance ally.

Tuesday night was clearly a reset for Vance. He proved that he’s capable of turning on the folksy charm that eluded him on the campaign trail, and that he could appeal to middle America as a potential leading man against future primary adversaries like Ron DeSantis or Glenn Youngkin. CNN’s snap poll showed his likability shooting up 19 points, from negative 22 points to negative 3—still underwater, but a sign of palatability. “He did himself a lot of good beyond that one night,” said a Republican consultant. “If he can pull off the MAGA banner in a non-offensive style that sells, then he could be the natural heir apparent, and that’s what you’re seeing start to gel here.” Another Republican operative noted the electoral value of Vance’s hybrid, country-club friendly MAGA approach that was once considered DeSantis’ lane. “This is the party. Get on board, or get the fuck out of the way,” this person said. “They think we’re all knuckle draggers and Neanderthals. J.D. Vance can debate any part of the establishment and can whip their ass.”

When he wasn’t on Truth Social, posting about the death of disgraced baseball legend Pete Rose, I’m told that Trump enjoyed the V.P. debate. While he might resent Vance for his strong performance, he was apparently overtaken with giddiness by Tim Walz’s general squirminess. (Even Democrats close to Kamala Harris admit that Walz was just “fine.”) For now, Trump sees Vance’s performance as affirmation that he chose wisely and Harris did not.

All that said, it’s not lost on those around Trump that they need to manage their glee over winning the news cycle, particularly since it was inelegantly displaced by the Jack Smith bombshell. “We understand that this debate doesn’t matter, but it was the much-needed win after the last debate,” said a campaign source, acknowledging the low morale. This being Trumpworld, of course, there’s an unspoken protocol regarding the appropriate amount of praise to heap on anyone besides the principal. “When you have so much staff radiating with glee, there’s a risk that it will irritate the old man,” the campaign insider continued. “No one is going around one-by-one praising J.D. to Trump.”

I was talking about this on twitter and thought would make a good post. At any rate, this year has really been a great year for haters. 50 cent and Kendrick Lamar really demonstrated that all of us have not hated anywhere near our full potential, but I will tell you someone else who is putting on a show this year- Liz Cheney.

That woman hates Donald Trump with a passion that is equal parts admirable and terrifying. She has made it her life’s work to make Donald Trump miserable until his dying breath, and she won’t stop there. I would not put it past her to hold a press conference after his funeral to correct the record if anyone says anything nice about him during the service. And then she’ll probably go after the kids and Kushner.

If you or I put half the energy she puts into hating Donald Trump into anything, think of what you could do. I could be the #1 pick in NBA draft if I devoted myself have as much to hoops as she puts into hating Trump. It’s fucking beautiful what she is doing. It’s a clinic on hating.

And the best part is it doesn’t cost Democrats a fucking thing. There is only upside. Yes, there are a few idiots who say GREAT EMBRACE A NEOCON, but we haven’t at all. Hell, half the Democrats when asked about her say “I don’t agree with a god damned thing that crazy lady thinks but I’m glad she’s voting for us.” We have not given up one thing, and Cheney does not give a shit and is out there campaigning and trashing Trump every chance she gets. It’s an amazing spectacle.

And the best part about it is that it makes people like Greenwald cry.
