Earlier today Russia did another double tap strike in Zaporizhzhia targeting rescue workers who’d responded to the initial strike.

Again a Russian double tap attack on Ukraine. This time on Zaporizhzhia. One missile strike, then, 40 minutes after, another one. Four people killed, many injured. Journalists who were on the scene to film the first attack, wounded too. Ukraine needs air defence urgently!

“These are people I served alongside in Ukraine, Turkey, Morocco, The Bahamas, Indonesia, Mexico, Gaza, and Israel. They were far more than heroes,” said WCK founder and Chief Feeding Officer José Andrés. “Their work was based on the simple belief that food is a universal human right. It is not conditional on being good or bad, rich or poor, left or right. We do not ask what religion you belong to. We just ask how many meals you need.”

WCK coordinated everything correctly with the IDF in advance. Despite that, our convoy was hit as it was leaving the Deir al-Balah warehouse, where the team had unloaded hundreds of tons of humanitarian food aid brought to Gaza on our newly established maritime route from Cyprus. “This is not only an attack against WCK, this is an attack on humanitarian organizations showing up in the most dire of situations where food is being used as a weapon of war. This is unforgivable,” said Erin Gore, CEO of WCK.

The IDF has acknowledged its responsibility and its fatal errors in the deadly attack on our convoy in Gaza. It is also taking disciplinary action against some of those in command and committed to other reforms. These are important steps forward. But without systemic change, there will be more military failures and more grieving families. In addition to the tens of thousands of Palestinians killed, nearly 200 humanitarian workers have lost their lives during this conflict. That is why World Central Kitchen is demanding the creation of an independent commission to investigate the killings of our WCK colleagues.

While we continue to demand full accountability for what happened to our team—our friends—we continue to mourn the loss of their incredible hearts and smiles. WCK’s leadership is in close contact with their families, and we are doing everything we can to support them in this unimaginable time. The campaign of independent presidential candidate Robert F. Kennedy Jr. sent out with a fundraising email Thursday referring to Jan. 6 defendants as “activists” who have been “stripped of their Constitutional liberties,” echoing former President Donald Trump’s rhetoric about the 2021 riot at the U.S. Capitol.

In the appeal sent to supporters, signed by “Team Kennedy,” the campaign called for the exoneration of “political prisoner Julian Assange,” the founder of WikiLeaks, suggesting he and Jan. 6 defendants are victims of prosecutorial abuse.

“This is the reality that every American Citizen faces — from Ed Snowden, to Julian Assange to the J6 activists sitting in a Washington DC jail cell stripped of their Constitutional liberties. Please help our campaign call out the illiberal actions of our very own government,” the fundraising email read…

After publication of this piece, Kennedy spokesperson Stephanie Spear told NBC News that the language was an “error.”

“That statement was an error that does not reflect Mr. Kennedy’s views. It was inserted by a new marketing contractor and slipped through the normal approval process,” she said…

As NBC News reported, there are just 15 Jan. 6 defendants who are currently being held in pretrial detention.

Most of them are credibly accused of violence against law enforcement officials…

One black couple did exactly that in 1939, and now, in 2024, generations later, we can see the impact of what that couple did with one brave, bold, and simple, act.  A black couple rented a home to a Chinese family.

A Black couple defied racism by renting to a Chinese family. Now comes $5 million in thanks (LA Times)

Every morning, Lloyd Dong Sr. would take the ferry from San Diego to Coronado, where he worked as a gardener for wealthy homeowners. And every night, he would retreat back home across the bay, barred by racially restrictive housing practices in the early 1900s from renting or buying his own house in the town.

Gus and Emma Thompson — a Black couple who had managed to secure ownership of Coronado property before restrictions took hold — boldly rented a house they owned to the Dong family, whose Chinese heritage blocked them from living in the community. The intersection of these two families amid the embedded racism of the time would decades later become a story of gratitude, made possible by the very home that once belonged to the Thompsons.

Some 85 years since the Dongs moved to Coronado, Lloyd Sr.’s sons, Ron Dong and Lloyd Dong Jr., are donating $5 million from their portion of the sale of the house they eventually came to own to San Diego State University’s Black Resource Center.

The gift will expand scholarships for Black students and fund future renovations at the center, its director, Brandon Gamble, said.

“I don’t know how to describe the feeling in my chest, but there’s a feeling that racism gives that folks are familiar with; you may not be able to describe it all the time,” Gamble said. “This is the complete opposite, and we don’t get to access it enough.”

Ron Dong, 86, the eldest son, said his father “tried and tried [to live in Coronado] and the only thing that came up was Gus Thompson willing to rent his house that he had available…. That was the big plus for our family, because it has made all the difference for us.”

Nudges to low deductible low cost plans in California

Earlier this week, JAMA Health Forum published a new study by Lovichikova et al on Covered California’s nudges to move people mid-year from bronze, gold and platinum plans to CSR 94 plans in the summer of 2021. As part of the the American Rescue Plan, two things happened mid-2021.  First, subsidies increased dramatically so that anyone with incomes under 150% Federal Poverty Level could buy a Silver CSR 94 plan with a ~$500 to $1000 deductible with no net premium.  Secondly, anyone collecting any unemployment income for 2021 would have their income deemed to be 150% FPL.  Covered California, the California state based marketplace, reached out to their current enrollees who they knew had some unemployment income.  This randomized control trial sees what the effect of an e-mail nudge is on enrollment and use.

The emails led to a statistically significant 3.1–percentage point (95% CI, 2.6-3.6 percentage points) increase in CSR silver 94 enrollment (a 74.8% relative increase) by July 31, 2021, and a 1.3–percentage point (95% CI, 0.2-2.4 percentage points) increase (a 2.3% relative increase) in practitioner visits by December 31, 2021. The emails had no detectable effect on prescription fills, emergency department visits, or hospitalizations.

What did I get from this study?

First, people don’t pay attention to policy changes.  The switching rate without an e-mail was about 4%.  The e-mail nudges moved the switching rate to 7% or about a 75% increase in the risk of switching.  I can almost guarantee you that a lot of people left at least a substantial amount of premium on the table as 30% of the sample has enrolled in either Gold or Platinum plans.  A switch within the same insurer to their Silver CSR 94 plan would be a substantial premium saver.  Now there might be odd cases where someone had already MOOPED out in February so the change in six months of premium might not be worth the extra new cost-sharing, but that is a very rare case.  Lots of money is being left on the table.  The nudge helps but it is limited relative to cross-checking of automatic re-assignment and re-enrollment.

Secondly, actuarial value is not suppressing demand as there is very little demand:

Absolute percentage point change in Silver CSR enrollment, and four types of healthcare utilization comparing individual households that received e-mails and those that did not. Clinician Visits significantly increased by ~1.2 percentage points. All other utilization insignificant changes.

People moved from lower actuarial value and higher deductible plans to higher actuarial value plans with substantially lower deductibles and cost-sharing:

in 2021, Covered California enrollees in a silver 94 plan paid $5 for a primary care visit, paid $3 for a generic prescription drug fill, and had an out-of-pocket maximum of $1000; by contrast, Covered California enrollees in a bronze plan paid $65 for a primary care visit, paid $18 for a generic prescription drug fill, and had an out-of-pocket maximum of $8200

Gold plans have a $35 PCP co-pay and $15 generic drug fill co-pay while Platinum plans would cost enrollees $15 to see their PCP and $5 to get a generic prescription. That is a big switch especially from Gold and Bronze plans to Silver CSR 94.

Yet there is no noticeable change in hospitalizations, ER use or prescription drugs and tiny changes in clinician use.  Cost sharing is not doing much which might be a bit weird if we approach the switching problem as an ordeal mechanism.  Ordeal mechanisms is an economics concept that says that burdens such as information gathering and compliance costs will sort the population into those who highly value a public benefit and those who don’t.  It is an allocation mechanism aiming to give a benefit to the most deserving/needy when there is substantial private information.  We would think that people who switch would have private information that their health is worse than average.  But we’re not seeing much evidence here to support selection by severity of need.

What are the policy pull-outs?

First thoughts:

A) Nudges work but not too much.

B)  Improving AV won’t change access to care that much

C)  Even among a population that had a demonstrated value of insurance, complex policy changes are complex and opaque.

 

I like this paper.  It asks a good question using a unique policy shock and good methods.

On Wednesday, the Council of Chief State School Officers announced that Testerman was chosen to receive the national title from among the state teachers of the year. First lady Jill Biden surprised Testerman during a nationally televised appearance Wednesday morning with a bouquet of flowers and the news that she and the nation’s other top teachers will be treated to a state dinner at the White House.

As Teacher of the Year, Testerman will spend a year traveling the country as an ambassador, urging other teachers to become strong advocates for their students and fellow educators.
